Description
Between the Supportive Housing Program and the Medicaid Redesign Team, 55 individuals are provided permanent supportive housing in scattered-site apartments throughout Erie County. Clients receive assistance in apartment search, treatment rehabilitation support, and independent community living skills.


Hours
Monday-Friday, 9am-5pm.

Eligibility
Individuals who have been referred to the program via the Erie County Single Point of Access (SPOA) referral. To qualify for SPOA, individuals have to be homeless or at-risk of homelessness and have a primary psychiatric diagnosis.

Service Area: Erie County.

Intake Process
All individuals are referred through the Erie County Single Point of Access (SPOA) system. This form is completed with the Coordinated Entry staff or a mental health provider.
