Description
A fully mobile team of mental health professionals providing a wide range of community-based treatment to individuals whose needs have not been met by more traditional mental health services.


Hours
Office: Monday-Friday, 8am-4pm.

Emergency on-call services available 24 hours a day, 7 days a week.

Eligibility
Adults ages 18 and older who meet eligibility requirements for Erie County Department of Mental Health Single Point of Entry (SPOE). Must have had prior inpatient treatment, including a minimum of two hospitalizations during the past two years.

Service Area: Erie County.

Intake Process
Complete Single Point of Entry application and submit to Erie County Department of Mental Health.

Program Fees
Medicaid.

English; interpretation services utilized as applicable.
