Intake Process
Call for appointment. Bring picture ID, proof of address, benefit and/or Social Security cards for family members, and proof of income.
Must bring the following to appointment:
- Picture identification (ex: Driver's License, Non-Driver's License)
- Proof of residency (utility bill, landlord statement, lease)
- Proof of number of people in household
- Proof of income
