Description
Offers care coordination and Cognitive Behavioral Intervention services to individuals being released from state prison or a local county jail to Erie County through the Enhanced Release Under Supervision Program. These programs assist individuals in reacclimating to the community while addressing behavioral and physical health needs, obtaining entitlements such as medical insurance, and decreasing recidivism.
